Parents / Caregivers are required to complete an enrolment form to enrol their child. To verify date of birth, parents need to show the birth certificate of children enrolling in prep or year one.
On the enrolment form, details of past and present medical problems (especially chronic illnesses such as asthma) are required to be completed. Discussions should take place between parents and teachers to ensure procedures are in place for treatment of any medical problems. It is strongly recommended that each child receive a complete medical examination prior to commencing school, with particular note being taken of sight and hearing. It is also strongly recommended that each child be immunised prior to starting school. A check with the doctor will identify which injections each child requires.
