How to enrol
To enrol your child at Koumala State School you will need to complete:
Application for Enrolment Form (PDF, 516 KB)
Consent Schedule (PDF, 102 KB)
Internet Consent Form (PDF, 28 KB
)
Minor Excursion Permission Form (PDF, 26 KB)
You will then need to bring these forms and documentary evidence of date of birth along to an enrolment meeting with administration at the school. If you cannot download the document linked from this page, please
contact us
and we will provide a copy via the school office.
